After signing up for our workers comp program, you can request worker's comp certificates under the "workers-comp-certificate-request" tab.
Certificate Request
The certificate of request section is for the company that is covered by OCMI. In order for us to find your file within our database, you will need to enter your company's information. The information needed is as follows: business name, address, city, state, phone number, and email address.
Certificate Holder
In the following section, we discuss the "Certificate Holder". Here you will need to fill out the client information, who is requesting the certificate.
Information required includes business name, address, city, state, phone number, and email address.
Additional information
Defining whether the client is requesting waiver of subrogation is the next step. Any additional information regarding the certification acquisition can also be included by the client.
